Breast augmentation
with lipofilling

Breast augmentation with lipofilling in Madrid and Albacete, breast augmentation with fat grafting or breast lipofilling performed by Dr. Miguel Fernandez Calderon is becoming more and more demanded. This treatment provides a very natural breast augmentation without scars. In this way, avoiding distortions in breast sensitivity and also including an improvement of the body contour.

It is not a technique as demanded as breast augmentation with implants because it is unknown. Also because it previously offered inconsistent results. Nowadays, following some basic principles, very similar results to those obtained with small breast implants can be achieved in one session.

Breast augmentation with lipofilling in Madrid and Albacete is one of the most demanded procedures in our clinic.

The advantages of this technique compared to the usual breast augmentation with breast implants are many. We can highlight the naturalness and the absence of significant scars. And also avoid complications related to breast implants.

The procedure is always performed under general anesthesia and usually on an outpatient basis. The technique to achieve predictable results must be meticulous. It also includes the extraction of fat from preoperatively marked donor areas, purification of the fat and infiltration following a multiplanar arrangement and in small amounts.

The pre-operative period for breast augmentation with lipofilling in Madrid and Albacete

Patients interested in breast augmentation with fat grafting or breast lipofilling should follow some general pre-operative recommendations. We explain them in the preoperative visit.

To achieve the best results in breast augmentation with lipofilling we always tell our patients these preoperative care in depth. This care together with the best technique in the operating room and postoperative care add all the ingredients to achieve the greatest satisfaction.

In order to have predictable results in breast augmentation with lipofilling, it is necessary that the patient meets some specific characteristics and clear objectives.

Preoperative care prior to the lipofilling breast augmentation procedure is extremely important. In order to achieve the best possible result.

1. Pre-procedure body fat reduction

The pre-procedure body fat reduction is essential to obtain the best result in breast augmentation with lipofilling. Most patients and many surgeons have a mistaken belief. Pre-procedure body fat gain will always work against positive final results.

During embryonic body development and to a lesser extent into adulthood the total amount of fat cells in the body changes. In adulthood, the increase in body fat does not translate into an increase in fat cells or adipocytes. Rather, they increase in volume but not in number. Thus, when we gain body fat there is an increase in the fat content of each fat cell but not in their number.

There can be a limiting factor in terms of the volume of fat graft infiltration that can be infiltrated into the breast. When it comes to breast augmentation with lipofilling, it is rarely the existence of inadequate donor sites. Usually the limiting factor is the distensibility of the breast and the infiltration planes. Thus, in one procedure, we can only infiltrate a certain volume of fat. Defined by these limiting parameters.

Adipocytes

This volume of fat will be composed of a certain amount of fat cells or adipocytes with the capacity to accumulate fat.

In order to obtain the best results, the objective when performing breast augmentation with lipofilling is to infiltrate the breast tissue with as many fat cells as possible, even if the fat cells are small in size. This is achieved by reducing the fat content in the body prior to the intervention.

In adulthood, when losing fat, the number of cells does not decrease, but the content of each cell decreases. And it is at that moment that the extraction is most effective. So that in the future, if we return to our initial weight, these fat cells can continue to accumulate fat inside and grow. And providing more volume to our breast graft. Once the fat graft has been incorporated into the normal breast tissue, it will increase in size without any problem as the body fat increases.

2. Proper hydration of the breast skin in the weeks prior to the breast lipofilling procedure

Moisturizing the skin of the breast prior to breast augmentation with lipofilling is essential to achieve the best results. The objective is to achieve the greatest possible distensibility in the breast coverage.

This allows to increase the volume of fat that can be transplanted in the intervention, without increasing excessively the pressure of the mammary compartment.

3. Use of external expansion devices

There are several external expansion devices on the market, such as the Brava system device, which achieve an increase in vascularization and, above all, an increase in the distensibility of the mammary skin. This allows us to increase the infiltration volume in the breast, without increasing excessively the pressure of the mammary compartment.

The disadvantage of these devices is that they must be worn continuously for at least 2 weeks prior to surgery. In addition to their high price, they are uncomfortable and difficult to use continuously during the working day.

Am I a candidate for breast augmentation with lipofilling breast augmentation in Madrid and Albacete?

Breast augmentation with lipofilling in Madrid and Albacete, is one of the most demanded specialties in our practice. The reason is that we are the best and most competitive specialists in this technique. Both for our theoretical knowledge and for our practical application.

It is a safe technique that can offer consistent results as long as it is performed on suitable patients and with a refined technique.

The most important factors when indicating breast augmentation with fat should be reviewed in the consultation and patients must meet them in order to have an expectation of favorable results. These factors are the following:

1. Balanced body weight

In order to achieve good long-term results of breast augmentation with lipofilling, the patients should have a balanced body weight over time. This is because it would not be indicated to perform lipofilling for breast augmentation in patients who plan to lose weight in the long or medium term. The reason is that the grafts will decrease in size over time and the result will worsen.

2. Defined shape with specific fat reserves that are difficult to remove.

Ideally, the procedure should be indicated in patients with small fat reserves that are difficult to remove. In general, these areas are usually the flanks, trochanteric region, inner thighs and knee. The more defined the area, the better the long-term result. the better the long-term result.

Breast lipofilling involves the tissue transplantation, the cells of which retain the fat reserve capacity they had prior to removal. Therefore, the more difficult it is to eliminate the localized fat in the donor area, the more difficult it will be to lose breast volume after the intervention, even if there is some weight variation.

3. Moderate or large breast skin distensibility.

The survival of fat grafting in breast augmentation with lipofilling in Madrid and Albacete depends on several factors. In addition to an adequate technique of extraction and infiltration, the right bed for the survival of the fat graft in the chest is essential.

The region where the fat is transplanted must be well vascularized. So the infiltration must be performed in planes that are rich in vascularity. In addition, the pressure in the compartment where the fat is infiltrated must not exceed the blood pressure of the small capillaries that will provide the nutrients for the graft to survive.

This compartmental pressure will increase when injecting little volume, when the skin of the breast does not distend well. However, in women who have suffered weight loss or after breastfeeding when there is more distensible skin, this pressure will increase more gradually. This will allow us to inject more fat graft.

Technique of abdominoplasty in breast augmentation with lipofilling in Madrid

In cases where the skin does not have sufficient distensibility, there are techniques that can increase it. These techniques can be external vacuum systems or by recruiting abdominal skin through the technique of reverse abdominoplasty with traction sutures.

Having good and predictable results in breast augmentation with lipofilling requires very important theoretical and practical knowledge. Everything begins in the first consultation, choosing correctly the patients who can benefit the most from this technique.

The factors described above are not essential to perform breast augmentation with lipofilling. But the more the characteristics of the patients are adapted to these requirements, the better results are expected.

Postoperative care in breast augmentation with breast lipofilling

Breast augmentation with lipofilling is a procedure with a high degree of satisfaction among patients.

There are numerous advantages of breast augmentation with lipofilling in Madrid and Albacete compared to breast augmentation with breast implants. Among these advantages we find the improvement of the body contour, since fat is extracted from regions where there is excess fat.

To optimize the results of breast augmentation with breast lipofilling, some requirements and preoperative care, the best surgical technique and simple postoperative care are necessary.

In the postoperative period of breast lipofilling it is essential to wear a compressive garment for the areas of fat removed for a month. It is also necessary that the patient follows the recommendations day by day to achieve the best result.

The final result of breast augmentation by lipofilling or fat grafting can be seen after approximately 3 months.

This operation is becoming more and more demanded and the reasons are clear. With this operation it is not possible to achieve the volume that can be achieved with breast implants in a single session. But using the technique correctly and following the recommendations it can offer moderate breast augmentation, with very natural results. Also without scars and with an improvement of the body contour.

Postoperative care of breast augmentation with lipofilling in Madrid and Albacete include:

1. Sleeping on your back for 4 weeks after the surgery

It is essential to avoid pressure on the chest for the next 4 weeks postoperatively. In order to get the best result in breast augmentation with lipofilling. It is therefore necessary to sleep on your back during these weeks which are the most uncomfortable of the postoperative period.

2. Use a compressive garment in the fat donor area and a bra that does not exert compression on the breasts.

During the first 4 postoperative weeks, it is necessary to wear a compression garment in the fat donor area. The garment is usually a girdle, since the most frequent liposuction areas are the flanks, thighs and abdomen. This is placed from below the chest to below the knee. This is done to avoid bleeding, reduce bruising, swelling and improve the aesthetic result of these areas.

In the chest it is necessary to wear a garment that does not compress excessively, except when we perform skin recruitment techniques (reverse abdominoplasty).

3. Postoperative massages

After the breast augmentation with lipofilling or breast lipofilling procedure, it is necessary to perform some massages in the liposuction area. This is done to optimize the result of body contouring. These basically lymphatic massages are intended to reduce swelling. And also to avoid irregularities and improve the contour.

These massages are usually started from the second postoperative week and are performed 2 or 3 times a week for 3 or 4 weeks.

4. Hydration in the breast region

After breast augmentation with lipofilling, there is a very important and sudden inflammation of the breast region.

This swelling leads to an increase in volume that can stretch the skin excessively. To avoid sequelae of this inflammation, it is very important to preserve a good state of hydration in the chest, using moisturizing creams, applied 3 times from the first postoperative day until the swelling subsides.

The moisturizing cream can be alternated with others to improve the resorption of the bruises. These will be applied both in the breast and in the liposuctioned areas.

5. Antibiotherapy and analgesia

The first few days after breast augmentation with lipofilling, it is normal to have some discomfort. Often in the form of stiffness, especially in the areas of liposuction. For this, analgesic medication and some rescue medication will be prescribed.

It is also necessary to take antibiotics for the first few days to avoid infections.

This medication is always prescribed personally after the operation.

Pre-operative methods to improve breast lipofilling results

Brava System

The Brava system is a continuous suction device. If worn for an estimated time it increases skin distensibility.

It is used to prepare the breast tissue to receive the fat graft. It is worn prior to breast augmentation surgery with lipofilling or breast lipofilling, in order to improve the results and viability of the fat graft.

The Brava system consists of two polyurethane domes. These are placed in the patient’s breasts together with a vacuum system. This causes an external pressure on the breast tissue. This suction causes an expansion of the breast tissue and increases its vascularisation.

This is held in position with a sports bra. It must be worn continuously for 2 to 3 weeks prior to the operation.

When worn, the external traction is felt and adjusted to be tolerable, without causing too much discomfort to the patient.

Sometimes the system can be used on its own to increase breast volume, but the increase is modest and not recommended. However, its most common use is to improve the capacity of the breast in breast augmentation with lipofilling or breast reconstruction through breast lipofilling.

REVERSE ABDOMINOPLASTY WITH FAT GRAFT OR RAFT (REVERSE ABDOMINOPLASTY WITH FAT GRAFT)

Reverse abdominoplasty with fat grafting is sometimes used to improve the technique of breast augmentation with lipofilling. It is performed in patients with poor breast distensibility and is also commonly used in reconstructive breast surgery. It is performed to define the breast sulcus, in autologous breast reconstruction techniques with fat grafting or reconstruction with flaps. And on occasion with breast implants.

The aim is to recruit abdominal skin to increase the volume of implantable fat in breast augmentation with lipofilling. And to improve its viability, as well as to define the breast sulcus.

Although it can be done in a variety of ways, we usually use a percutaneous technique to pass a special suture. This will recruit the abdominal skin upwards, including it in the breast defining the sulcus and also increasing the space to place the fat graft.
